Here’s a phrase you might not expect to hear in a dental office: “Let’s keep an eye on that.” When we spot an early soft spot on a tooth, drilling isn’t always our first move. Sometimes the smartest, gentlest thing we can do is catch it early and help the tooth heal itself. It sounds almost too good to be true — but early decay doesn’t always have to mean a filling.
Teeth aren’t static — they’re always in flux. Every day, your enamel loses minerals when it’s under acid attack and gains them back when conditions are right. That rebuilding process is called remineralization. When decay is caught early — as a weakened, chalky spot before it’s become an actual hole — we have a real chance to tip that balance back toward healing instead of immediately reaching for the handpiece.
What “catching it early” lets us do. When we find a spot in its earliest stage, we have gentle options:
- Ozone to disrupt the bacteria driving the decay
- Remineralizing support like MI Paste (calcium and phosphate) and hydroxyapatite to rebuild weakened enamel
- A closer look at the habits feeding the problem — diet, dry mouth, pH, home care
- Monitoring over time to make sure the spot is holding steady or reversing, not progressing
Often, that combination means a spot can be arrested or even healed — and a filling avoided altogether.
An honest caveat. This isn’t a promise that we never fill cavities. Once decay has broken through into a true cavity, or when it’s active and spreading, a filling is the right and necessary call — and waiting would only let it get worse. The “watch and remineralize” approach is for early spots, caught before that point. That’s exactly why we’re so proactive about finding things early, using tools like 3D imaging and careful exams to spot trouble while it’s still reversible.
Why this is biological dentistry at its best. Our whole philosophy is minimally invasive — do only what’s truly needed, and preserve as much of your natural tooth as possible. Every spot we can heal instead of drill is tooth structure that stays strong for life. The earlier we catch something, the less we have to do about it — and that’s a win for you and your smile.
So if we ever say “let’s watch that,” don’t worry that we’re ignoring a problem. We’re giving your tooth the chance to do what it’s beautifully designed to do: heal.
